Continuum of Care System

The Continuum of Care system is the mechanism used to move homeless from the streets to permanent, safe housing. Not all homeless move through the system in the same way, nor does every homeless person they need all of the components of the system. The fundamental components of a CoC system are:

    • Outreach, intake and assessment
    • Emergency shelter
    • Transitional housing
    • Permanent housing
    • Supportive Services
    • Prevention

 

Continuum of Care Plan  

According to the U.S. Department of Housing and Urban Development (HUD), “A Continuum of Care Plan is a community plan to organize and deliver housing and services to meet the specific needs of people who are homeless as they move to stable housing and maximum self-sufficiency. It includes action steps to end homelessness and to prevent a return to homelessness.”

 

The Continuum of Care Plan is the culmination of significant research and planning efforts. Macomb County Continuum of Care’s Plan is reviewed and submitted to HUD yearly.

 

Our CoC Plan is NOT the same as the 10 Year Plan to End Chronic Homelessness in Macomb County. The 10 Year Plan addresses one sub-population of the homeless. It does guide the development of chronic homeless goals in the Continuum of Care Plan.

 

Importance of a Continuum of Care Plan

A Continuum of Care and the plan that it develops are important for several reasons, including that it:

    • Provides for the assessment of capacity
    • Requires the identification of gaps
    • Makes the community become proactive rather than reactive
    • Helps the community develop a common vision and a set of common goals
    • Identifies ways of coordinating and linking resources to avoid duplication
    • Provides a forum to obtain community buy-in and access to mainstream resources
    • Allows the community to apply for certain federal and state grants
