Continuum of Care
Continuum of Care System The Continuum of Care system is the mechanism used to move homeless from the streets to permanent, safe housing. Not all homeless move through the system in the same way, nor does every homeless person they need all of the components of the system. The fundamental components of a CoC system are:
Continuum of Care Plan According to the U.S. Department of Housing and Urban Development (HUD), “A Continuum of Care Plan is a community plan to organize and deliver housing and services to meet the specific needs of people who are homeless as they move to stable housing and maximum self-sufficiency. It includes action steps to end homelessness and to prevent a return to homelessness.”
The Continuum of Care Plan is the culmination of significant research and planning efforts. Macomb County Continuum of Care’s Plan is reviewed and submitted to HUD yearly.
Our CoC Plan is NOT the same as the 10 Year Plan to End Chronic Homelessness in Macomb County. The 10 Year Plan addresses one sub-population of the homeless. It does guide the development of chronic homeless goals in the Continuum of Care Plan.
