Social Work Services


Martha T. Berry provides social work services to residents and their families to promote social and psychological well-being and quality of life throughout their stay at the facility. The social workers are responsible for ensuring that residents retain their individuality, independence, dignity and choice. Each resident will have a primary social worker available to provide advocacy, support, assistance and referrals from pre-admission through discharge.

    • ongoing emotional support and guidance, during both the admission adjustment period and throughout the entire stay
    • referral assistance for guardianship and conservatorship
    • education on “Resident Rights & Responsibilities” and all facility policies
    • participation in each resident’s care conference and in the development of a comprehensive care plan
    • grief counseling and crisis intervention to residents experiencing a difficult life event
    • assessment and referral to mental health service providers as needed
    • support groups available for residents with multiple sclerosis, Parkinson’s disease or other neurological disorders
    • referrals for hospice services
    • coordination of discharge planning process, including assistance in obtaining all follow-up services such as skilled home care, medical equipment, supplies, chore services, financial services, legal aid and Meals-on-Wheels
