At Martha T. Berry, we understand how difficult it can be to make the transition from hospital or home to a skilled nursing facility after an illness or injury. Many residents are admitted having difficulty with walking, caring for themselves, communicating, swallowing, or many other problems. To ease the transition, the Martha T. Berry rehabilitation team works with residents to become as functional and independent as possible. Residents gradually improve their strength and physical functioning while receiving excellent nursing care – maximizing their abilities and maintaining their dignity.
Our goal is for our residents to reach the highest level of function possible.
We work with the resident, family and the Social Services Department to coordinate our resident’s discharge as soon as he/she has reached his/her rehabilitation goals.
