Or you may print the required business form on your fax machine. If you are calling from within Michigan, call 888-99-CLERK. If you are calling from out-of-state, call 310-575-5035. Request document 3150 for a Sole Proprietorship certificate with instructions. Request document 3155 for a Co-Partnership certificate with instructions.
