1. Request for Hearing on a Motion
This is the local form that is used to place a case on the motion docket for hearing by a judge on a motion day. It can also be used as the notice of hearing and proof of service. It is called a Request for Hearing on a Motion. For an Adobe PDF version, click here. For a Word version, click here.
2. Family Division Confidential Asset and Liability Disclosure Sheet
The Family Division requires in the initial order that all parties complete these sheets and bring them to the first conference. For an Adobe PDF version, click here.
3. Facilitation Order.
This is the order which is used to refer cases to facilitation, a form of voluntary alternative dispute resolution utilized by the Court on a regular basis. For an Adobe PDF version, click here.

5. Indigent Appointed Attorneys' Billing Form
This is the form attorneys who are appointed to represent indigent criminal defendants in our Court are to submit to Judicial Aide for payment. Use of the form greatly increases the efficiency of bill processing. We recommend that counsel make a copy, keep in it their file and fill in the services they perform as each case proceeds. 6. Indigent Attorney Fee Schedule
This is the schedule that controls, as a general rule, payment of attorneys who are appointed to represent indigent defendants in the Macomb County Circuit Court. Click here.

8. Case Evaluator Application
This document is the application which attorneys who are interested in becoming case evaluators under MCR 2.403 should complete and file with the Case Evaluation/ADR Clerks. 11. Interpreters
Macomb County Circuit Court provides language interpreters for those individuals that cannot understand English or those who are unable to communicate due to a hearing impairment.
Court Interpreters are officers of the Court. As such, there are various procedures and rules of ethics which they must follow to preserve due process.
